Dedication
This book is dedicated to my family, who had to put up with me during all the hours spent writing and reviewing this manuscript over the years and all the time away.
Thanks to all the physician assistant students and physician assistants who have attended my board review courses over the years; without all of you this book would not have been possible, and thank you for all the suggestions that helped to make the book better.

The on-line examination has been reviewed and updated. These questions will provide you with a real-life
